Social Prescribing Project

What is Social Prescribing?

Social Prescribing is a one year trial project in Darlington.

It aims to support individuals to improve their wellbeing without the need for a medical or social care intervention.

Referrals are received by a team made up of representatives from four charities in Darlington; Age UK Darlington, Darlington Mind, Darlington Association on Disability and 700 Club.

A member of this team, known as a “Social Prescribing Broker” will work with the individual to…

  • Identify their support needs
  • identify services that might be able to meet these needs
  • signpost or refer the individual to these services
  • help the individual engage with appropriate services

· attend activities or appointments with the individual initially to help build confidence and establish engagement

· follow up with the individual regularly to see if their support needs are being met

Am I eligible?

You can be referred to the project if you…

  • have 3 or more long-term conditions
  • frequently use A&E, urgent care or GP services
  • have poor social relationships and or loneliness
  • are frail and/or elderly

How do I access it?

You can be referred to the project by…

  • your GP
  • your nurse or nurse practitioner
  • your Social Worker
  • any other health or social care professional

This project is jointly funded between NHS Darlington CCG and Darlington Borough Council.
